Been trying to figure this one out for my garage band. Just worked thru it yesterday with my bass teacher.

Intro - C

Verse 1 & 2
C Gm Bflat F (2x)

Eflat-maj7 (no 3) Bflat F Csus/E (3x)

Interlude - Gm

Verses 3 & 4 only use the 2nd chord structure above throughout

Outro - C until final Gm

Note: the Eflat-maj7 (no 3) chord can also be written as Bflat/Eflat (ie a Bflat chord with Eflat as the bass note)

Maybe my ear is off, but that F#-maj7 sounds like a plain old F to me...

I noticed in a video that Cooley plays this with a Capo on the fifth fret.
First two lines: A Em G D
Rest of song: C G D A

Patterson doesn't play much on guitar for this, and Neff does the slide work...
